Service Description:
Early Support is a service designed to deliver better coordinated service for parents/carers of disabled children aged 0-5, as soon as the child is identified as needing additional support.
Provides families with a single point of contact and continuity through coordinated working enabling services and professionals to work together with the child and family's needs at the centre.
Referrals can be made by family/carers/heath or social care practioners.
